Types of Medical Decision Making (MDM)

MDM refers to the cognitive process healthcare providers engage in when making clinical decisions for patient care. It is categorized into four levels based on the complexity and risk involved. These levels include:

Straightforward: This level involves minimal complexity, such as a routine follow-up visit or straightforward management of a known condition.

Low: Low complexity MDM typically includes a single, stable chronic illness with mild exacerbation or a new problem with a limited diagnostic workup.

Moderate: Moderate complexity MDM may involve multiple chronic illnesses, acute illness with systemic symptoms, or an undiagnosed new problem with a moderate diagnostic workup.

High: High complexity MDM encompasses multiple complex chronic illnesses, acute or chronic illnesses with severe exacerbation, or a new problem requiring extensive diagnostic workup.

Modifiers are two-digit codes added to the Current Procedural Terminology (CPT) codes to provide additional information or alter the definition of the service provided. Here, we will define three commonly used modifiers:

Modifier 24: This modifier indicates that an unrelated evaluation and management (E/M) service was provided by the same physician or healthcare professional during a postoperative period. It signifies that the E/M service was distinct and unrelated to the surgical procedure.

Modifier 25: Modifier 25 is used to indicate that an E/M service was performed on the same day as a separate procedure or other service. It identifies that the E/M service was significant and separately identifiable from the other service provided.

Modifier 57: Modifier 57 is used to indicate that a decision for surgery was made during an E/M service visit, leading to the performance of major surgery on the same or following day. It emphasizes the significant role of the E/M visit in the decision-making process for the surgery.
