Expressing education on how they will get home, so they are prepared when the discharge comes at 8 am on their discharge day. Also offering Lyft to people who don’t have any sort of transportation. Another part that needs to start at admission is education about when you go home and how you will continue to take care of yourself. So that when it is time for discharge, they are comfortable about being sent home. Finally, control of implementing the new way to discharge and adjusting as needed to improve the discharge process.
