Most recently in the organization I work for we have transitioned a medical surgical floor to a telemetry floor. The necessary steps for successful change are to increase the urgency, build the guiding team, get the vision right, communicate for buy-in, empower for action, create short-term wins, do not let up, and make change stick (Kotter, 2002). This change came about because we had an increase in the number of patients coming to the hospital with a need for telemetry monitoring with not enough beds to care for these patients. These telemetry patients were having waiting periods in our emergency department that sometimes lasted for days. To meet the needs of our patient population, and increase patient satisfaction, our senior leadership team got together and devised a plan to convert one of our medical surgical floors that did not have a full census to a telemetry floor. This was a major change and required extensive planning, education, and communication with our nurses and nursing union to facilitate this needed change.
