Pulmonary embolism (PE) and deep vein thrombosis (DVT) are serious medical conditions that can lead to significant morbidity and mortality if not diagnosed and treated promptly

QUESTION

PICOT question

In patients with shortness of breath or leg pain (P), is the D-dimer test (I), compared to CT/Ultrasound (C), more accurate for identifying the presence of deep vein thrombosis (DVT) or pulmonary embolism (PE) (O), over a 6-month trial period (T).

  1. Develop an EBP Standard – Describe two to three interventions (or a bundle of care) from the evidence and discuss how individual patient preferences or the preferences of others will be considered.
  2. Implications – Summarize the potential contributions of the proposed project for nursing research, education and practice.

INTRODUCTION This is what the paper is about.
Pulmonary embolism (PE) is caused when a blood clot occurs in the lung causing shortness of breath. Deep vein thrombosis (DVT) is when a blood clot occurs in the leg causing leg pain (U.S. National Library of Medicine, 2020). A timely and accurate diagnosis is critical for proper care and the prevention of consequences. In the examination of suspected DVT or PE, the D-dimer test and imaging modalities like as CT (computed tomography) and ultrasound are routinely used (U.S. National Library of Medicine, 2020). Both disorders cause severe morbidity and mortality if they’re misdiagnosed or left untreated (Kostantinides et al., 2019). Patients with DVT suffer from pain and swelling, that causes inability to walk or perform activities of daily living.  On the other hand, patients with PE can suffer from lung damage making it hard for them to breathe (CDC, 2023). Early and correct diagnosis is critical in providing appropriate treatment and avoiding problems. Over a 6-month trial period, this initiative will compare the accuracy of the D-dimer test and CT/ultrasound in detecting the existence of DVT or PE.

The scholarly search sites that were used to obtain PICOT study material were CINHAL Complete, PubMed Central, Web of Science databases, ProQuest Central, and PubMed/MEDLINE search engines through the United States University Library. A couple of Google scholar articles searches also were used. The key terms used to guide the search for the evidence were “d-dimer test,” “CT, “ultrasound,” “deep vein thrombosis (DVT),” “pulmonary embolism (PE),” “shortness of breath,” and “leg pain.”

ANSWER

Introduction

Pulmonary embolism (PE) and deep vein thrombosis (DVT) are serious medical conditions that can lead to significant morbidity and mortality if not diagnosed and treated promptly. PE occurs when a blood clot travels to the lung, causing shortness of breath, while DVT is characterized by blood clots forming in the deep veins of the leg, leading to leg pain and swelling (U.S. National Library of Medicine, 2020). Accurate and timely diagnosis is crucial to prevent adverse outcomes, and various diagnostic methods are employed, including the D-dimer test and imaging modalities such as computed tomography (CT) and ultrasound.

PICOT Question

In patients with shortness of breath or leg pain (P), is the D-dimer test (I), compared to CT/Ultrasound (C), more accurate for identifying the presence of deep vein thrombosis (DVT) or pulmonary embolism (PE) (O), over a 6-month trial period (T)?

EBP Standard

Two potential interventions or a bundle of care to consider from the evidence are as follows:
Integrated Diagnostic Approach:Implementing an integrated approach combining the D-dimer test and CT/ultrasound could provide a comprehensive assessment of DVT or PE. This approach can leverage the sensitivity of the D-dimer test as a screening tool and the specificity of imaging modalities for confirmatory diagnosis. Involving patients in shared decision-making can be integral, discussing the benefits and potential risks of both methods, considering patient preferences and clinical indications.

Patient-Centered Education: Develop educational materials and programs to inform patients about the importance of early recognition of symptoms, seeking timely medical attention, and understanding the diagnostic options available. Empowering patients with knowledge about the significance of their symptoms and diagnostic tools enhances their involvement in the diagnostic process, aligning with patient-centered care principles.

Implications

The proposed project holds significant implications for nursing research, education, and practice. From a research perspective, it contributes to the evidence base by directly comparing the accuracy of the D-dimer test and CT/ultrasound in detecting DVT and PE. This research can inform clinical guidelines and policies, providing clinicians with evidence-based recommendations for diagnostic strategies.

In terms of nursing education, the project underscores the importance of integrating evidence-based practices into nursing curricula. Educating nursing students and practicing nurses about the advantages and limitations of different diagnostic approaches fosters critical thinking skills and equips them to provide patient-centered care.

Regarding nursing practice, the project’s findings can directly influence clinical decision-making. Nurses can play a pivotal role in educating patients about the diagnostic options available, facilitating informed decision-making, and addressing patient concerns. By considering patient preferences, nurses can contribute to the individualized care plan that aligns with patients‘ values and needs.

In conclusion, the project’s interventions and potential contributions underscore the critical role of nurses in enhancing patient outcomes through evidence-based practice, patient engagement, and integrated care approaches. This project has the potential to reshape nursing practices, improve patient experiences, and foster collaboration among healthcare professionals for optimal diagnostic outcomes.
