Every year large number of patients are harmed or die due to avoidable harms such as healthcare-associated infections, falls, wrong-site surgeries, medication errors, pressure injuries, and so on. This unsafe care is increasing the burden of death and disabilities globally. Evidence suggests 134 million adverse events occur due to unsafe care in hospitals in low and middle-income countries, and that accounts for 2.6 million deaths every year (World Health Organization [WHO], 2021). The social cost of harm is estimated to be 1 to 2 trillion a year (WHO, 2021). It is documented that most of this harm is avoidable. Patient safety incidents not only cause death and disability but also causes suffering for their families. Financial and economic costs of safety issues are high.  Serious patient safety concerns reduce public trust in the healthcare system. Patients may suffer from lasting psychological injuries followed by an incident that may have resulted in disability or injury.”

The good news is that over the past 20 years, healthcare organizations have begun to realize and accept that most errors cannot be linked to the performance of the individual and are instead the result of a series of system failures. The even better news is that these system failures are often preventable (Berwick, n.d.)

“Nowadays, I hope that health care is maturing into a better kind of system for a worker like me — frail, error-prone, human.” ~Dr. Don Berwick (IHI). Another scenario as a good lesson learned is the wrong surgery site: A patient is scheduled for surgery on her left leg. Initially, an intern prepares the patient by marking the correct surgical site on the dressing on the leg.

The intern’s teammate removes the dressing but addresses the problem by marking the surgical site on the patient’s skin. However, he uses a water-soluble marker. The ink becomes smeared and illegible.

Our professor’s post has great points on the importance of selecting the right methodology because selecting the wrong methodology for any given project can result in errors.
