QUESTION

Watching this video made me incredibly angry. We go into healthcare to help people and it is people like those in this video that should see their way out of bedside, Our patients are depending on us to care for them in their time of need, not cast judgment and disrespect their culture. The nurse should have started the interaction by taking her time and not rushing so she could go on break. She needed to be more understanding post-op when caring for Mr. Pierre and not ignore his call light. Some hospitals allow for homeopathic remedies to be used for pain management, if more pain medication was not able to be administered, the nurse could have initiated them. There are many instances of stereotyping and bias in this clip. During patient registration, the registration employee makes the statement that the patient is African American, when corrected that the patient is actually Haitian, she states "What’s it matter?" This is unacceptable. The nurse stereotypes the patient as being a "baby when it comes to pain" due to his culture. Even if you have a selection of people from a specific culture who do not tolerate pain well, that does not mean you ignore their calls for help. Something was seriously wrong with Mr. Pierre and it could have been prevented if she paid attention to her patient instead of generalizing him based on his culture. In the clip, the staff member who is taking the patient to his procedure is whistling, in the Haitian culture, this is disrespectful to do to elders. When explained, the employee rolled his eyes, which is unacceptable. This is an example of ethnocentrism. The employee does not understand the Haitian culture so their view of whistling is foolish, putting his culture ahead of the patients. There was no assimilation in this clip. The patient’s culture was never taken into account in a positive way. Even the statement about how the patient couldn’t have stew from home because the nurse didn’t want the unit to smell like curry. Not only was that uncalled for and frankly rude, but it demonstrates that she wants the patient to comply with her culture. An example of acculturation is the oxygen post-op. The nurse could have explained better to Mr. Pierre’s son why the oxygen was needed to see if he would keep it on, not say he had no choice just because the patient did not understand. Our patients depend on us for care when they are not able to care for themselves. It is our duty to include their culture in that care for the betterment of the patient. Quest Top! ANSWER

The video described in the post has triggered strong emotions and a sense of anger in the viewer, which is understandable given the evident disregard for patient-centered care and cultural sensitivity displayed by some healthcare professionals in the depicted scenarios. It is indeed disheartening when individuals in healthcare, who are expected to provide compassionate and culturally competent care, fall short of these expectations.

Patient-Centered Care: The primary objective of healthcare should always be to provide patient-centered care. Healthcare professionals must prioritize the well-being and comfort of their patients, understanding that each patient is unique, and their cultural background can significantly influence their healthcare needs and preferences. Rushing through interactions or neglecting patient requests can have detrimental effects on patient outcomes.

Cultural Sensitivity: Cultural competence and sensitivity are fundamental in healthcare. Healthcare providers should strive to understand, respect, and honor the diverse cultural backgrounds and beliefs of their patients. Stereotyping, bias, or making sweeping generalizations about patients based on their culture is not only disrespectful but can also lead to suboptimal care. It is crucial to approach each patient with an open mind and a willingness to learn about their cultural preferences.

Communication: Effective communication is a cornerstone of quality healthcare. Patients and their families must be able to communicate their needs and concerns. Dismissive or unresponsive behavior, such as rolling one’s eyes when cultural differences are explained, is not only unprofessional but can also lead to misunderstandings and mismanagement of care.

Ethnocentrism: Ethnocentrism, or the belief in the superiority of one’s culture over others, has no place in healthcare. Healthcare professionals should avoid imposing their cultural beliefs on patients and instead seek to understand and accommodate the cultural beliefs and practices of their patients, provided they do not compromise patient safety.

Cultural Inclusivity: Inclusive care involves recognizing and embracing the cultural backgrounds and traditions of patients. This includes considering dietary preferences, cultural practices, and traditions that can positively impact a patient’s well-being and comfort during their healthcare journey.

Acculturation: Acculturation, or the process of adapting to a new culture, can be challenging for patients and their families. Healthcare providers should support patients in this process, providing clear explanations and options rather than dictating decisions.

In conclusion, the viewer’s anger and frustration in response to the video are entirely justified. The situations depicted in the video highlight the critical importance of cultural competence, patient-centered care, and respectful communication in healthcare. These principles are not only ethical but also essential for achieving positive patient outcomes and ensuring that all patients receive the quality care they deserve, regardless of their cultural background. It is incumbent upon healthcare organizations and professionals to continually educate themselves and prioritize cultural competence in their practice to provide the best care possible to all patients.
